Request Procedure
1. A request for access to Haywood de Bruyn Attorneys must be made.
2. by completing the prescribed form C, which is available on the website www.sahrc.org.za; or
3. in terms of the regulations, which can be found on the website www.doj.gov.za
Such request must be sent or delivered to one of the addresses referred to in contact details and must be directed to Jarome Haywood.

the request must:
- provide sufficient detail on the request form to enable Haywood De Bruyn Attorneys to identify the record and the requester;
- indicate which form of access is required;
- state the manner to be used to inform the requester as to the availability of the record;
- provide the necessary contact particulars in order to be so informed;
- identify the right that is sought to be exercised or protected;
- provide an explanation as to why the requested record is required for the exercise or protection of that right; and
- if the request is made on behalf of another person, submit proof of the capacity in which the requester is making the request to the satisfaction of the Managing Partner;
- A requester who seeks access to a record containing personal information about the requester is not required to pay the prescribed request fee;
- Every requester who is not a personal requester must pay the request fee of R100.00.
- The requester may lodge an application to the court against the tender or payment of the request fee;
- The requester will be notified by Haywood De Bruyn Attorneys in the required manner when a decision has been made on the request;
- If the request is granted, the requester must pay a further access fee for the search, reproduction and preparation of the record, as well as for any time that has exceeded the prescribed hours to search and prepare the record for disclosure.
